What do you mean?: my first question is what do you mean chemotherapy fail. what kind of symptoms the patient has to indicate failure. and to answer your question, if surgery is no longer an option to control the metastases. then it is wait and see.

Selected instances: Metastatic colon cancer when in multiple areas and non resectable reequires chemotherapy. Radiation can not be used systemically and is delivered to a single site. If mass is large, in preparation for resection, radiation has been employed. New monoclonals being developed thatt have alpha emmitters and can be delivered to colon Ca as a form of internal radiation.

Yes: While chemo like FOLFOX is transiently effective, more immunotherapeutic agents are showing good responses. Avastin (bevacizumab) a vascular endothelial growth factor inhibitor and Erbitux effecting epidermal growth factor 1 are working well as is the newer Neo-102 against the immunogenic protein. Chemo in conjuction with the biologics (immunochemo) is the best combo.

Modestly effective: Fluoropyrimidines (5-FU, Capecitabine and UFT) as a class have modest activity and one of these 3 needs to be combined with Oxaliplatin and or Irinotecan to achieve best results ,as combination chemo is far superior in efficacy than any single chemo drug
