Ensure: Is your cold underweight? If he is underweight try to increase his calories intake by eating healthy nutritious food, small frequent healthy meals can add up. Ensure is high in sugars & that can cause the vomiting, talk to his pediatrician, he might give a plan for his diet or you could a dietitian. Good luck .

Possibilities: He could be allergic or intolerant to one or more of the components of Ensure.He could also have an unrelated illness that coincides with your introduction of this product. The first issue resolves if you quit giving it, the second would not. I would return to his previous diet and consult with his doc on how to proceed.
