Can be Muscle strain: OR can be a referred pain. Did you have any injury or fall or sudden pull of Right Arm/ neck? Or do you have cough and fever with this? As it is going on for a week and not responding to NSAID/Ive or Moist heat; then it is time to get examined by your doctor:-)

Inflammation: You may have pleurisy which is inflammation of the lining over your lungs due to many reasons, including infections from viruses etc. Try a short course of Advil (ibuprofen) and Vicks vaporub inhalations a few times daily. If fever or no improvement, you may need to be checked by your doctor for the need to get a chest X-ray.
