Can be Muscle strain: OR can be a referred pain. Did you have any injury or fall or sudden pull of Right Arm/ neck? Or do you have cough and fever with this? As it is going on for a week and not responding to NSAID/Ive or Moist heat; then it is time to get examined by your doctor:-)

Inflammation: You may have pleurisy which is inflammation of the lining over your lungs due to many reasons, including infections from viruses etc. Try a short course of Advil (ibuprofen) and Vicks vaporub inhalations a few times daily. If fever or no improvement, you may need to be checked by your doctor for the need to get a chest X-ray.

Neck/shoulder pain: Most of the time the neck and shoulder pain result form muscle, arthritis and nerve involvement as it emerges out of the vertebral column, Need exam and may be some testing to get to the diagnosis, see your primary care doctor

Time to get seen!: This could be due to pleurisy, pneumonia, a blood clot or even a pneumothorax (a portion of your lung collapsed), as will as to an infection in your abdomen or pelvis. It could also be no big deal, but there's no way to tell without seeing you. So you've been sick long enough and it's time to get an answer and treatment. Go to your doc, an Urgent Care or an ER today and get it sorted out.
