Arthritis: Mild spondylosis = mild aging changes (aka arthritis). Tiny disc protrusion is just what it sounds like. Small right paracentral osteophyte = small bone spur just to the right. With a tiny disk protrusion, I'm betting the report says "without" significant thecal sac deformity, which means no significant pressure on the spinal cord. In a 47 yo patient, all this would be considered normal.
