Mirena (levonorgestrel) bleeding: Irregular bleeding can occur with Mirena (levonorgestrel). There is no safety issue specific to having sex while bleeding. The IUD works the same whether you are bleeding or not. The risk of catching a sexually transmitted disease is present but not increased when bleeding on an IUD
