My urethral opening is about as large as my vaginal opening. what can i do to fix this? also how does this happen? i am also wondering if the fact that i don't use my urethra to urinate at all(i have a mitrofanoff) could be a factor in this). thank you !

Hard to say: Without knowing more of your medical history, it would be hard to say exactly why this is going on. A mitrofanoff procedure is a connection between your bladder and abdominal skin, using your appendix, which tells me that you may have had difficulty emptying your bladder in the past. Catheterization and weak pelvic musculature may thus contribute to this finding. You need to see a urogynecologist.
