Many possibilities: There are many things that can cause abdominal and back pain including problems with your back, muscle problems, GI problems, endometriosis, and urinary tract problems. Your doctor will evaluate all these possibilities in an orderly fashion to try to get you an answer as fast as possible. Hope this helps.

Vaginal & back pain: Other possible diagnosis would be endometriosis. This can be missed on ultrasound and goal diagnosis would be an explorative laporatotomy. Usually pain is cyclic along with periods.
