Tooth Abcess: your teeth are natural part of your body. Yes a tooth abscess can get into the bloodstream and actually spread into your whole body. Need to see a dentist ASAP.

Yes: Before antibiotics were introduced, tooth infections were one of the leading causes of death. See a dentist to have that tooth examined and treated.

Very rare.. But...: Unless you have damaged or replaced heart valves an infection of the heart from the teeth is quite unusual. If valves are damaged you do need to be extra diligent about your dental health. Dental infections more commonly spread and become life threatening if they get into the throat ( closed the airway) or brain. Very severe infections can lead to a general sepsis, but this is less common.
