How to switch tasks when hyper-focusing while on adhd medication?

Use a timer: If you want to be sure to leave one task to start another, set a time limit on the former and use a timer to remind you to switch. Another option if studying, for example, or working on a school project is to work with a friend or classmate. Make plans to meet to carry out the task and be sure to show up.
