Wart or callous?: Hard to say based on your description. Strange for a kid to get a single callous on pinky toe, although I suppose it is possible. It is more likely a type of wart, for which an OTC salycilic acid preparation may be used. Neither callouses nor warts "swell." They do grow in size; but if there's swelling or redness, that is inflammation, possibly related to infection, which should be evaluated.
