You might need: a surgical repair. If you do not treat you ight get a frozen shoulder, less painful but more disabling

Surgery: Thank you for your question! Sounds like you are in a lot of pain. You have done just about all of the conservative therapies that are available. I would suggest that you see an orthopedic surgeon.
