how long is it supposed to it take for plaquenil (hydroxychloroquine) to get rid of the lupus butterfly rash?

Give it 6 months: Plaquenil (hydroxychloroquine) takes up to about 6 months to reach its maximum effect. It often has little or no benefit in the first 6 weeks of its use.
