See your PCP: The first thing you need to do is make sure it's gout. See your PCP for consultation, tests and diagnosis. Colchicine is taken orally to relieve an acute gout attack.

Steroid injection: There is a concern with severe pain and inflammation of the joint for two weeks. My experience after 34 years is to be aggressive with an intra articular steroid injection. Additionally a change in an nsaid , preferably another class than the amides
