Dr have 4yo zpak for 5 days for bronchitis and sinus infection. he wrote it for 5ml po qd, isn't the firt day suppose to be doubled? then 4 days lower

Antibiotic: you are correct that day 1 is double the dose and the next 4 days would be half that dose, however in certain circumstances we do give the higher dose for 5 days or even 10 days. it is safe

Doesn't matter: The med you are citing could be given all at one time or spread out in several different ways. All at once increases the chance of stomach irritation & potential for vomiting in kids. The med attaches itself to fat cells in your body and gradually enters the blood over a week or more. Extra given early just loads the fat cells, not the treatment. By the end of treatment it comes out equal.
