Tough question: I have always felt that the risk of plaquenil (hydroxychloroquine) is less than the risk of lupus flaring up but a very tough call. Depends on the severity of lupus, antibody status, pt age, etc. Also no guarantee that a flare will respond to plaquenil (hydroxychloroquine) 2nd time around. I usually recommend continuing 200mg a day indefinitely.
