Many things: These types of white matter changes are COMMONLY seen in otherwise perfectly healthy individuals who undergo MR scans for headaches & a variety of other symptoms. Frequently, no cause is determined. These findings might be consistent with Lyme disease, prior head trauma or concussion, prior brain infection or inflammation and vasculitis, amongst others.

ABN MRI: ABNORMAL FINDINGS ON BRAIN MRI NEED TO BE CORELATED WITH THE CLINICAL EXAM AND LABS. The differential DX would also include CNS LYME/CNS SJOGRENS SYNDROME/CNS SARCOIDOSIS . It would be wise to get a second Neurologic Opninion and Rheumatolgic opinion. A lumbar puncture may be needed to evaluate CNS LYME. A pronged period of respiratory failure with hypoxia may also cause these lesions.
