Is epi-pen critical while taking allergy drops? doctor said it's a precaution but that none of his (hundreds) patients have ever needed it. $$$

Yes: Any time you do immunotherapy from home it is important to have Epinephrine on hand in case of an allergic reaction. That being said, their are only a few FDA approved sublingual tabs on the market. Allergy drops are often unregulated and insufficient. Drops that contain multiple allergens could be a waste of time as there is only so much surface area under tongue for absorption. Check into it.
