Could be eczema: The red rash in the back of an infant could be eczema or other form of inflammatory condition. In either case it needs to be closely followed by the baby's physician.

Possible: A rash needs to be seen to diagnose it properly. you don't state how long it's been there and if is causing discomfort such as itching. would get it evaluated again by his doctor or a skin specialist
