Is it rare for a undiagnosed heart defect, previously unknown, to present for the first time as heart failure in a 20 y/o :) is it unlikely yes or no?

Rare but possible: There are certain heart defects which may not be evident until early adulthood such as septal defects or noncompaction cardiomyopathies. Other heart conditions such a viral cardiomyopathy can occur in any age.
