Many many things: but predominantly hundreds of organic compounds collectively known as "tars," which are aromatic hydrocarbons that are largely responsible for the inflammatory nature of smoking consequences, including bronchitis and emphysema, and its propensity to cause cancers. In addition, there is carbon monoxide and formaldehyde in the smoke, and hundreds of other compounds of unknown safety in humans.
