Needs attention: If this is a new symptom , it could be a sprain Or other muscle inflammation. If it is a chronic problem there are many more considerations. He should have a good medical evaluation in either situation.

Pain: alternating pain in the neck and shoulder of a child is not a typical finding. An evaluation by your pediatrician would be the appropriate first step. It would be important to know how long this has been occurring? Does it happen every time he runs? Any recent trauma? Any other medical issues? More questions need to be answered before a proper assessment can be provided.
