been reading about hoarseness after total thyroidectomy so basically even with the best surgeon you can have mild hoarseness or voice issues after?

Not really: hoarseness associated with a total thyroidectomy is usually related to either injury to or inflammation around the recurrent laryngeal nerve. It can be up to 10% risk of either temporary or permanent nerve damage. All the literature suggests that the most important factor in deciding a surgeon is volume. I only send my patients to high volume thyroid surgeons.
