Several possibilitie: May just be natural curvature or size for you, but carotid tumors or aneurysms are possible. Pain is concerning but could it be referred from jaw or teeth? A quick non-invasive test would be an ultrasound that is painless and relatively quick and could answer your questions. Would check with vascular or cardiac doctor to arrange such a study.
