GVHD is a reaction..: of engrafted immune cells against the patients healthy tissues. It leads to inflammation and side effects in a variety of tissues. Rejection is the opposite. It is a reaction of the patients immune system against the engrafted cells. In the case of a bone marrow transplant, rejection would cause disappearance of the cells in the marrow and a drop in blood cell production.
