My five month old has been scratching her left ear for about a month now. what is the cause? she even does it in her sleep. no temperature.

Multiple possible: By 5 mo, an infant can put their hand where they want. This can be to signal discomfort in the ear (pressure.infection) ,referred discomfort of teething, an itchy feeling in the ear, etc. In the absence of fever or mood change, I would not assume it is very important.
