Need more informatio: The spots that you describe as blood spots need to be evaluated. I can't tell from your description as to whether the blood spots are tender themselves or are these dilated vessels, are there areas of loss of tissue, or are there neurologic issues involved,so we need more information. These could be an area of trauma depending on your shoe

Small red spots: on side of the toe can be number of things, including infection, fungus issue or even cold injury. You would be helped if you see a foot doctor for a better evaluation and treatment.
