Metformin is 1st: Metformin is first line. It is weight neutral to some mild weight loss potential, does not generally cause hypoglycemia, and reduces insulin resistance. It can cause some loose stool as a side effect and has a fishy smell. Also if sick in the hospital or dehydrated, you should stop the medication temporarily to avoid the very rare side effect of lactic acidosis. It is inexpensive.
