Mgt/workup: He should be seen and evaluated to determine the underlying cause. From your description, screening labs should include a complete metabolic panel and CBC, diff, plts and ESR. His parents should also request a complete neurologic exam, an exam for swollen nodes and a specialist if the cause can not be determined.
