It was determined i have heterogeneously hypoechoic nodule 1.8 x 1.2 x 1.0 cm. there is internal vascularity, with no microcalcification. concerned?

Needs biopsy: I am not sure where this nodule is located, breast or thyroid? The finding is non-specific. Nonetheless, the size of the nodule warrants a biopsy. If the nodule is located in the breast or thyroid, it can easily be biopsied with ultrasound guidance.

Depends: It depends where this nodule is (thyroid or breast, etc), other characteristics of the nodule (such as well or ill-defined margins) and whether this nodule has been seen before or not.
