Various causes: This discomfort could be from the chest wall, heart, lung, esophagus, or the cartilage structure at the junction of the rib and sternum. Tension and stress can play a role. In other words your physician will have to take a thorough history and perform a thorough physical examination in order to make a diagnosis. Appropriate tests will be ordered.
