Dermatologic: It's could be a dermatologic pathology. Sounds more like a contact dermatitis. Did she wear some new socks or shoes three weeks ago. I would have her see a dermatologist or podiatrist for evaluation.

Stasis Dermatitis: She could have stasis dermatitis as a result of fluid overload. This can happen if the valves of the veins do a poor job of sending the blood up the leg and back to the heart. Usually starts with swelling, and then a red, itchy rash may develop where the skin is stretching (can be around entire ankle, but usually on the inside mostly). See a podiatrist.
