Adhesions likely: If your pain is directly related to the deep breath, that indicates that movement of your diaphragm is causing a problem. If you have ever had surgery or even infection of the abdomen or pelvis, adhesions may form and these may create pain because of nerve fibers trapped in the scarring(adhesions) so that the deep breath may affect those rigid areas and cause the pain on that side.
