Many options: Shoulder is mainly a soft tissue joint, not a lot of bony contact. Likely AC joint, possible arthritis changes, could be rotator cuff tendon sliding b/t humeral head and AC joint, old Labrum injury, Arthritis of Gleno-humeral jt itself. Physical exam by a doctor would help to diagnose.
