Sharp back pain: Most probably, the pain you describe is muscular or myofascial -- a sudden cramp due to muscle tension. Remote other possibilities include viral pleurisy (would worsen with breathing in), pulmonary embolism (atypical for PE and unlikely), pinched nerve (not likely either), or referred from biliary stone (rare with no abd pain). Try massage and heat. Get checked if it moves or worsens
