Thats up to you: You are entitled to a copy of your medical records no questions asked. The office may charge you a reasonable amount to make copies. Whether or not your previous doctor will know about the request or not is probably determined by the size of the office. If it makes you feel better to tell them why you are leaving then do so, but you do not owe them any explanation.

No need: The Privacy Rule gives you the right to inspect, review, and receive a copy of your medical records and billing records that are held by your health care provider.
