Nerve pain: Your pain with a bowel movement is the stool putting pressure on your bowel in the area before the end that has nerve compression with referral of pain to your back. Referred pain with bowel movements is not uncommon as long as the patient has had it for a long time and the discomfort is not changing over time. It is difficult to make this go away except for with time.

GI specialist needed: I am sorry to hear that you have had constant constipation. The mainstay of treatment of constipation is that of Fiber (fiber one cereal), Water, exercise, and trials of stool softeners (ex sodium docusate). I would say that you should speak to a GI specialist about your issues. Maybe it is due to Gastroparesis or some other gastric motility problem. A GI specialist would be able to best help you
