May be normal: Infection is a possibility but less likely 3 weeks out. An exam would be helpful.

Maybe nothing: It could mean you just overdid things and it is a little swollen but if you have any signs of infection like increasing redness or fever or heat, you might want to check with your surgeon.

Stitch: Usually it is a stitch that wants to come out from the inside. If it is pain and swell (edema) then there is an inflammation going on and this is the first usual cause. Your OB should take care of the problem. Sometimes some other stitches might come out too.
