I have red spots under my feet. it's very painful. however, it is not itchy and i also have diabetes. i take metformin and i have insulin please help?

I would suggest : You visit a foot doctor. Most dermatological lesions are not described as painful. The fact you are stating you have pain is a concern. Since, you are diabetic you are also a high risk patient. An in office evaluation is more prudent than an online guess. Please see a doctor in person.
