Rarely: About one in 10000 patients may experience mild liver toxicity (expressed as elevates ALT & AST ).

Best approach: Metformin is often prescribed for conditions in which liver enzymes rise (the non-alcoholic fatty liver family), and when these are present, the enzymes can rise late. I'd only conclude metformin is the cause if hepatitis B & C, Wilson's, alcohol, other meds (ibuproten can do it), autoimmunity are ruled out, liver's not fatty, and withdrawing metformin resolves this.
