It can be: depends on the the level of exposure. A person who is in a room with a single smoker has one level of exposure, and someone in an enclosed and unventillated place with many smokers will have an increased level of exposure. Depends also on how deeply you are breathing, or how rapidly. I have seen many patients with significant second-hand smoke exposure have positive cotinine tests.
