Anal fissure: A colonoscopy can rule out additional diagnosis, but does not treat with fissure or hemorrhoid. An exam of rectum under anesthesia with fissure repair and hemorrhoid removal could be of benefit. Ask for surgical referral/consult. Take a friend with you to hear the risk/benefit analysis and details of supporting you through recovery. Good luck.
