Yes and no: You can kill all the germs with 24 hours of antibiotics but the reason for taking antibiotics for 10 days with strep is that it takes that long to get rid of any risk of developing post streptococcal rheumatic fever. Yes, you can have strep without while patches. It is even possible to have strep without even having a sore throat, but that would be unusual.
