It depends on : Severity...Right sided blood is not well oxygenated...If this blood bypasses the lungs it will not get oxygenated. If severe enough this will cause hypoxia.

Shunt: R to l shunting moves deoxygenated blood into the systemic circulation so less oxygen gets delivered to the tissues. It also makes it possible for venous clots to get into the arterial system. This is not good.
