Need culture: Sure sounds suspicious for a urinary tract infection, but without a urine culture that is all just speculation. Nitrites show up when bacteria are present, and you may not have lots of wbc's with some infections, so you really need to get a urine culture ordered to know for sure you don't have an infection.

Need further eval: You should have a thorough check up by a generalist. I would especially want to know about your lymph node and abdom organ exam. Labs should be looking at blood count and srum chemistries- especially kidney function and protein level. Many things come to mind but i would start with that to direct further workup.
