No easy answer: We can only speculate. First, i don't see a connnection between ncs and altitude induced chest pain. But several things occur at altitude: the oxygen content of air is lower so even though your stress tests don't show significant blockage, you may have enough blockage to be significant at 3, 000 feet. Also, just as people get migraine at altitude, coronary spasm can also be provoked at elevation.

Syncope: There are multiple possible causes for what's happening to you above 3k feet. Gas expands as you go up and you may have GI or pulmonary gas expansion causing your chest pain. At 3k feet there still is good oxygenation in the blood so that shouldn't be in play unless you have pulmonary disease or anemia. If you had signficant coronary problem it likely would have shown on the stress echos.
