This is serious: Ongoing thoughts about doing harm to oneself is a very serious matter and probably indicates severe depression. If you have a psychiatrist, contact that physician; otherwise, let your primary care physician know. You need to be in a safe environment where you will be protected from doing harm to yourself. You may also require medications or other treatments to help you get better.

Call 911 . . . : If you become imminently suicidal. Otherwise make an appointment with your psychiatrist to discuss treatments for severe depression. There are several different treatments beyond antidepressant medications, if those are exhausted. Persist until you start feeling and behaving better.

ECT: For patients who have failed numerous antidepressants and other "talking" therapies and remain acutely suicidal, electro convulsive therapy (ECT) should be considered. With success rates as high as 95%, this form of treatment has saved many lives. Not all psychiatrists perform ect. One may contact their local psychiatric association for referrals. Many university hospitals have ECT programs.
