Chest pain: Lots of causes for left upper back pain. There is cardiac, pulmonary, musculoskeletal and gastrointestinal potential causation. You'll need to consult your doc to work out the cause of your particular pain.

L scapula: The bone in your back behind your heart is the scapula - it's generally not involved in heart pain. Unless you only get it with lower extremity exertion (like walking), i'd look for a local cause: shoulder arthritis, bursitis, tendonitis or cervical spondylitis.
