Maybe neither.: The muscle that separates the abdomen from the chest is the diaphragm. When it is irritated it can sometimes cause pain in the shoulder ( right side to right shoulder and vice versa). An ectopic may not irritate the diaphragm so that there may be no shoulder pain at all.
