Shoulder impingement: Last option should be surgery. Physical therapy for posture principles for neck and shoulder as well as scapular stabilizer muscle and rotator cuff muscle strengthening. This should reestablish the normal scapulohumeral motion and eliminate impingement.

Sometimes: Surgery for impingement should be a last resort if conservative measures have failed. Nsaids, physical therapy and possible cortisone injections are the initial treatments.
