Depends: Treatment would depend on diagnosis of problem. Some of your problem sounds like it may be due to a neuroma.

Neuroma formation: Metatarsalgia is pain under the metatarsal heads. Sometimes the nerve between the metatarsal bones gets pinched and becomes a painful neuroma. I assume the insoles did not help. I recommend evaluation by a podiatrist. Other possible causes are painful metatarsophalangeal joints from arthritis.
